Change Management in Semantic Web Services Change Management in Semantic Web Services in Legal Domain using FSM XXM Introduction With the new paradigm of Service Oriented Computing, many enterprises attempt to utilize services as fundamental elements for developing applications/solutions as an additive layer on top of existing components. The Web Service Controller Architecture (WSCA) for service-based, enterprise-scaled business solutions provides exactly this flexibility. The design, exposure and management of services can be accomplished through a Service Oriented Architecture (SOA) that supports the usage, composition and coordination of services in a loosely coupled manner. Web services appear to be particularly suitable for addressing these aspects of a SOA. Furthermore, composition languages such as BPEL add value by composing Web services and facilitating the implementation of business processes. As the SOA paradigm brings this big behavior change relying on loose coupling of services it raises new questions in the area of change management. Change management is a central aspect in any software design, but its complexity for Web services is increased by both composition languages and loose coupling. The resulting advantages like composability, autonomy, message-based communication, and the avoidance of combined compilation and binding prove to be deficiencies in this context. In this paper we discuss these elementary challenges of change management in the area of Web services (Web Service Change Management, WSCM). Currently there are no sound mechanisms and engineering principles for changing Web services and their related entities. Through analysis of a suitable scenario, specifically looking at the details of the Web service provider and consumer roles, one can identify the various problems that exist in this domain. Therefore we will start our approach with the consideration of an application scenario from the business domain of application and change management, the addition of Web services in private legal sector. An Overview of a WSCA A WSCA consists of several autonomous outsourced Web services, but acts as a virtually coherent entity. Business entities, in the form of Web services, are often geographically distributed and organizationally independent. While WSCA has a potential to introduce new business opportunities through dynamic alliances, the challenges of fully realizing a WSCA lie in managing changes during its lifecycle through Extreme X Machines (XXM). XXM: In this paper we explore the design changes as made to software projects by the use  of a formal model known as Extreme X-Machines (XXM) [5,6], which  are based on the work of Eilenberg and Holcombe [7,8]. An XXM model describes the functionality of the software without defining exactly how this functionality is achieved. This perspective allows an analysis of functional change whilst excluding specific implementation or requirements issues. XXM are a state based model, they are intended to be used by developers as a method to design  their systems from at the top level but here, XXM are used as effectively to analyze and incorporate changes in, even addition of new web services. Each model typically consists of a set of states which correspond to screens in the final system and functions which link the screens together. The functions are typically labeled with an enabling action such as â€Å"click_ok† which corresponds to a user clicking the OK button. Figure depicts the architecture of a WSCA. There are two key components and two supporting components in this architecture. The key components include a WSCA schema and a WSCA instance. The two supporting components include ontology providers and Web service providers. †¢ WSCA schema: A WSCA schema consists of a set of abstract services and the relationships among these services. An abstract service specifies one type of functionality provided by the Web services. They are not bounded to any concrete services. They are defined in terms of service concepts in a Web service ontology. †¢WSCA instance: A WSCA instance is a composition of a set of concrete services, which instantiates a WSCA schema. It actually delivers the functionality and performance of a WSCA. †¢ Ontology providers: The ontology provider manages and maintains a set of ontologies that describe the semantics of Web services. A WSCA outsources semantics from an ontology provider to build up its schema. †¢ Web service providers: The Web service providers offer a set of Web services, which can be outsourced to form WSCA instances. The lifecycle of a WSCA is a series of stages through which it passes from its inception to its termination. There are four phases in a WSCA lifecycle: initiation, composition, service-orchestration and termination. The initiation phase is the first stage, where the WSCA is described at a high level. It is initiated when the owner of the WSCA establishes a need for a business objective [53]. The composition phase deals with integrating the selected Web services [42]. After this, the selected Web services are orchestrated to provide the value-added service. The termination phase occurs when the owner of the WSCA decides that the execution of the WSCA is no longer required. To materialize the concept of WSCA, the WSCA must automatically adapt to its dynamic environment, i.e., to deal with changes during its lifetime. Because of the dynamic nature of Web service infrastructure, changes should be considered as the rule and managed in a structured and systematic way [52]. Changes may be introduced by the occurrence of new market interests, new business regulation, or underlying service availability. Such changes require a corresponding modification of the WSCA structure with respect to the functionality it provides and the performance it delivers. Once a change occurs, a WSCA must react in a reasonable time and realign itself to deal with the change. This alignment must be performed in an automatic manner considering the frequent occurrence of the changes to a WSCA. By doing this, the WSCA can not only deal with unanticipated changes to the underlying services and infrastructure, but also maximize its market value, optimize functionality outsourcing, and maintain competitiveness. Problem Formulation The problem with the existing system of legal decision making process is that: There is no universal coalition body to unite the various modules of law in private sector like: A Public Interface to query for a case Online allotment of Advocates to users Judgments Union of India (to export Central Acts and Amendments) State Acts (to export State Acts and Amendments) The whole system of legal enactment starting from user query, allotment of Advocate, and referring judgments is not under a single roof. Due to inadequacy of getting the latest and updated Acts or Amendments, sometimes old acts are cited which later cause problems when the case is taken up in courts. This is a cause of embarrassment to the lawyer. In the existing System there is no spontaneous notification to the legal bodies regarding a change in the Acts. They themselves have to search for it on internet or find them in the latest issues. The problem is that they are not notified about the change in Acts automatically. Proposed Solution The need is to have an automated Legal System in India, at least at the private level. And this automation should be done with the most contemporary and best suited group of technologies. The problem uncovers the major issue of incapability of notification of changed Acts or Amendments to the related bodies. The proposed WSCA model namely LegalCosmos functions to provide a comprehensive Legal Solution that outsources the functionality from various service providers, such as WS_IndianUnionAmmendments, WS_StateActAmmendments, WS_PublicInterface, WS_AdvocateAssociation, WS_Judgments (depicted in Figure 1.2). The user in need of a legal solution makes use of the web service WS_PublicInterface and submits his, here accused, details and the details of the FIR filed by the complainant and forwards the FIR details like Case Crime No, Name, crime, date etc., to the WSCA which allots a Lawyer for the case with the help of WS_AdvocatesAssociation. An interested user (may be lawyer or a normal user) can find the details of the judgments through the WS_Judgments service. When a judgment is made the WS_Judgment web service provides the user with the copy of the order of the judgment. When these services are combined together as a WSCA, the WSCA will invoke the services on behalf of the user. There may potentially be some dependency relationship between them. These dependencies determine the composition of the services. In the case of the LegalCosmos WSCA, users do not necessarily need to provide the information for each service. The input of some services can be derived from the dependency relationship. Like in WS_AdvocateAssociation selected advocate details are directly passed to the WS-Judgments. Fig. Change Reaction for adding two new web services WS_UnionOfIndia, WS_StateActs WSCA Schema using Finite State Machine Definition: FSMis a mathematical  model of computation  used to design both  computer programs  and  sequential logic  circuits. It is conceived as an  abstract machine  that can be in one of a finite number of  states. The machine is in only one state at a time; the state it is in at any given time is called the  current state. It can change from one state to another when initiated by a triggering event or condition; this is called atransition. A particular FSM is defined by a list of its states, and the triggering condition for each transition. Fig. WSCA Schema (A simple XXM) Change Management in LegalCosmos The change management in the proposed LegalCosmos WSCA primarily deals with the change in Acts in the form of Amendments. This change origins from the addition of two web services provided by the â€Å"Union of India† that produces amendments at the central level and the â€Å"State Acts† that produces amendments at the State level. Since these changes are initiated by the outsourced service providers and incorporate an alteration in web service. They form the part of bottom up changes. The bottom-up approach for managing changes is highly dependent on the services that compose the WSCA. Therefore, it is necessary to first define the changes that occur to Web services, and then map them onto the business level. These changes include Activation changes that occur at the service level and WSCA changes that are executed at the business level. Finally, we provide rules for mapping triggering changes to their respective reactive changes. In our work, we assume that activation changes occur asynchronously. For instance, the WS_PublicInterface service may not change its data types while the activation change of unavailability is being managed. Another assumption we make is that the service is associated with a set of states. We associate each change with a transition between two states: precondition and postcondition. For example, a precondition for PI’s unavailability is that it was previously available and the postcondition is that it has become unavailable. Activation changes and their respective preconditions and postconditions will later be used to model changes using FSM and XXM. Fig. Activation Changes Non-Functional Changes Non-functional changes represent the dependability, usability, and trust associated with a member service. This information may be stored by a third party, the WSCA, or member services. We assume the information is stored as attributes that are maintained by an independent third party service provider. Changes to the trustworthiness of a Web service relate to the security, reputation, and privacy of a Web service. Changes in usability are dependent on the responsiveness and service cost. Finally, changes to dependability are associated with the availability and reliability of the Web service. Changes to service dependability are binary, because they represent two distinct states. For example, a service may either be available or unavailable. Alternatively, changes to service trust and usability are non-binary. For instance, service cost may assume more than two values during a member service’s lifetime. Therefore, we assume that a threshold is set and maintained by the WSCA. This threshold declares the minimum and maximum intervals of a value accepted by the WSCA. For example, the WSCA has the threshold of minimum $5 and maximum $10 for any judgment service cost. Every time a change occurs to the cost of a member judgment service, it is compared with the threshold. Only if the change exceeds the threshold, we consider that a triggering change has occurred. Note that the changes we have defined, such as changes to availability, are applicable to member services only. Once a member service is replaced, it is no longer part of the change management mechanism. For example, the e-Acts member service EA1 may become unavailable, and prompt the WSCA LegalCosmos to select an alternate e-Acts service EA2. After some time, EA1 may become available. However, since it is no longer a part of the Lega lCosmos LCS, the LegalCosmos WSCA does not manage the change in EA1. Table 6.1.1.1 summarizes the non-functional changes in Web services. Non-Functional Changes Fig. Non-Functional Changes Functional Changes Unlike non-functional changes, which are based on attributes, functional changes deal with changes to a service’s WSDL description [22]. We represent functional changes as a combined execution of a remove followed by an add. We further classify functional changes into structural and behavioral changes (Figure 6.1). Structural changes refer to the operational aspects of a Web service. For example, a structural change in a PI service can be caused by changing the operations offered to a user. Changes to the behavior of a Web service are indicated by changing its interaction with external entities. Functional changes to a member Web service occur when its WSDL description is modified. We assume these changes are initiated by the service provider. Fig. Functional Changes XXM for Existing LegalCosmos WSCA, when Functional Changes are not introduced (before the removal of e-Acts web service) The key functional changes involved here are: Removal of e-Acts web service (E-Copy of acts) shown in dashed rectangle. It has to be removed as it gives only immutable acts and does not involve latest amendments done by the Bare Acts. This causes problems to the WSCA in maintaining latest updates of the amendments and sometimes leads to false application of acts. A new web service called WS_UnionOfIndia is added to the WSCA which supplies the latest acts and amendments to WSCA. The Union of India is responsible for making and amending acts on the Central level. A new web service called WS_StateActs is added to the WSCA which supplies the latest acts and amendments to WSCA. The State Acts is responsible for making and amending acts on the State level. XXM for WSCA when Functional Changes are introduced (after the removal of e-Acts web service and addition of two new web services, WS_UnionOfIndia and WS_StateActs) Fig. The above figure shows the two newly added web services namely WS_UnionOfIndia and WS_StateActs. WSCA Changes Each activation change will initiate a WSCA change at the business layer. WSCA changes may occur at the composition and service orchestration levels of a WSCA. For instance, a Dt change in the PI service may trigger inconsistencies in the WSCA composition, such as incorrect parameter data types. Alternatively, a Da change may disrupt WSCA service orchestration. We divide the WSCA changes into composition and service orchestration. Fig. WSCA Changes at business level Mapping of Changes A Mapping specifies how changes in one layer correspond to changes in another layer [86]. These mappings must remain consistent in the presence of frequent changes. When a change occurs at the service level, the business layer must react to manage the changes. Triggering changes have a reactive impact on the business layer of the WSCA. Our approach of mapping changes is based on mapping rules. These rules are based on the activation changes and their corresponding business level changes of WSCA. Some changes may have more than one rule associated with them. Come Back to the Bush Analysis Essay

The poem â€Å"come back to the bush† is an Australian poem written by Thomas a spencer. The poem describes Australia and the Australian outback and how incomparable it is to the city life of London. The poem explores why the outback is unique and great and creates an image in the readers mind representing the Australian outback. The man in the poem has left his home in the outback to go live the high life in London. The verse â€Å"so I brought my folks to London, and we’ve settled down in style†, gives the expression the man is ecstatic and overjoyed living in London. Although the poem explores this particular emotion of the persona, the composer is yet to reveal the personas ‘rough Australian outback man’. This side of him has not left him and voice haunts him to come back home â€Å"to the bush and the wallaby track, to the home in the clearing, the sheep and the sheering†. For those who have not experienced the Australian outback the poet may be perplexing (confusing). The outback is very harsh and barren; the Australian men who have lived in the outback are made for the desolate terrain. The outback is one like no other and has a special connection to many who reside there. This connection has been made with the man. In the poem the voice represented is that of the man who talks about his life in London and how he is living a wonderful life in London. The man later talks about the Australian outback calling him back home. This crushes him and he is baffled with what to do next. The poem addresses the man and talks about how the man is reminded of his home country and where he belongs. The poem addresses men who have left the outback for the city life. The poem calls to men and women who are being haunted on making the life changing decision on whether to leave or stay in the outback. The man has made his decisions but his now reconsidering what to do. The poem shows the distress the man is in and how unaware he is on what to do. The situations the words are spoken in are helplessness, regret and confusion. The persona is regretful of the move and is helpless on making the decision on what to do next. The main argument in the poem is should the man go back home to the outback and why the man had left and weather it was the right decision to make. The reader of the poem is given the perspective that the man is regretting his decision and the question is asked to the audience, is it alright to leave your home and who you are for money and a high life? The reader is quiet involved in the poem which is a very effective strategy used my Thomas spencer. In the poem the reader is invited to see the effect of leaving your identity behind. We as the readers see the regret in the man and his helplessness towards the decision he had made and how that decision had impacted his life. The poem is very meaning full and is interesting. It relates to many readers and is a great poem with the theme belonging embedded in the poem.

Barriers to Accessing Health Care Services

In this essay I am going to critically analyse diversity within the National Health Service (NHS. I will briefly focus on barriers to accessing health care services in relation to age, race, disability, gender and culture and sexual orientation. I will consider the barriers which have unintentionally been put into place within different health services for both service users and members of staff, and the measures that have been taken in order to redress these issues by the government, NHS and Department of Health (DOH). Diversity is the inclusion of all irrespective of an individual’s age, race, capabilities, cultural background, gender or sexual orientation. The St. Helens and Knowsley NHS trust affirm this ideal describing diversity as, â€Å"Different individuals valuing each other regardless of skin, intellect, talents or years. † This is a quote the St. Helen’s and Knowsley trust have used from the Equality Act 2000 on their website to show their beliefs surrounding diversity. In society every individual is born differently, such as hair colour, skin colour, male or female, sexual orientation (depending on your belief that sexual orientation is genetic and not environmental), born into different religious and cultural backgrounds. As the U. K. has become ever more a multi-cultural society, issues such as diversity and anti-discriminatory practices have become ever more significant within our society. An individual who has been treated differently or received an unequal quality of service based on a preconceived idea due to a minority group they may identify themselves with have been discriminated against. Within the NHS these minority groups are known as characteristics, as in characteristics which help build an individual’s idea of their own self-concept. Since the Equality Act was up-dated in 2010 the NHS have not only included race, culture, gender, capabilities, religious beliefs and sexual orientation but also now include, pregnancy/ maternity, marriage/civil partnerships and carers as part of their protected characteristics policy. (http://www. nhs. uk). For the first time the law also protects people who are at risk of discrimination by association or perception. This could include, for example, a carer who cares for a disabled person† (http://www. nhs. uk) There are two forms which discrimination can take place the first is called direct discrimination. Direct discrimination is when an individual is treated differently usually negatively or unfairly compared to others based on the individual identifying themself with one of the above protected characteristics. Indirect discrimination is when a law, policy or procedure has been put into place, which applies to everyone, but this law, policy or procedure will disadvantage individuals who associate or identify themselves with the above protected characteristics. However due to organisations focusing too much on anti-discriminatory practice a new controversial concept of positive discrimination has begun to emerge, this is also being called positive/ affirmative action. Positive discrimination is where minorities or individuals, who associate or identify themselves with the protected characteristics, are given preferential treatment to others. An example of this would be two candidates who are both equally qualified for a job, however one candidate is a white, heterosexual male and the other candidate is a black, homosexual female, by positive discrimination the second candidate would receive the position. Positive discrimination can be due to pressure felt by the organisation to employ a diverse work force, to fill a quota to ensure the organisation cannot be accused of being discriminatory or historical guilt for issues such slavery. â€Å"The idea of positive discrimination came from the US where an ‘affirmative action’ programme has been used to try and ensure the make-up of certain workplaces reflects that of society. † (http://www. findlaw. co. uk) It has been suggested that the root cause of discrimination is due to prejudice. Prejudice is a stereotyped, pre-conceived idea of the way an individual or social group should portray themselves within society (Walsh et al, 2005). For example all Irish people drink alcohol excessively. â€Å"Prejudices can be a result of your own beliefs and values, which can often come into conflict with work situations. † (Pg. 194, Nolan et al, 2005). There are no laws in place against being actively prejudice, this is why the law concentrates against discriminatory practices. (Walsh et al, 2005. ) Ageism is commonly thought of as discrimination against the elderly; many nursing and health and social care textbooks affirm this belief, Kydd et al (2009, Pg. 49) state that â€Å"Ageism is the generalisation of old age as a social problem, this has contributed to the negative stereotyping and ageist practices in relation to older people. † However this is a false perception. Ageism can affect individuals of all ages, for example in 2004 the upper age limitations of free breast screening were 69, (http://www. imsersomayores. sic) however due to a growing older population this age restriction was raised to 73 in 2010 and the lower age limitation remained at 47. (http://www. cancerscreening. nhs. uk). Studies carried by the cancer research charity show â€Å"The 5 Most Commonly Diagnosed Cancers in Females, Average Percentages and Numbers of New Cases, by Age, UK, 2007-2009† (http://www. cancerresearchuk. org). Breast cancer is highest in both categories of women aged 2 5-49 and 50-74, however in the group 25-49 breast cancer was more prevalent by 10% (43%) compare to the 34% in the group of women aged 50-74. These figures show there is a need for the lower age restriction to be reduced. This NHS policy for age restrictions on breast screening needs to be reviewed as it can be viewed to be discriminatory towards women in the UK under 47 who are proven to be in need of this health service. Racism is the belief that one race is superior to another; (walsh et al, 2005) throughout history millions have faced discrimination and persecution due to this belief the most prevalent examples of this is in the Jewish community and the Black community. As the UK continues to become a more diverse, multi- cultural society racism continues to plague Britain. Penketh (2000, Pg. 7) affirms this by stating, â€Å"Black people are more likely to be ‘stopped and searched’, arrested, imprisoned and even to die in custody than whites. † Studies carried out by Unison, the UK’s largest healthcare trades union found that 70% of their non-Caucasian client’s employed by the NHS had experienced ‘racism or racial discrimination’ at work, some employees had experienced verbal abuse and physical abuse in relation to their race. (http://www. unison. org. uk). From this study the NHS have brought out an initiative to help eradicate racism within the NHS, part of this initiative is to have a better complaints and redress system in place, as three quarters of the employees in this study who experienced racism or racial discrimination were unsatisfied with the outcome when the incident was reported to their management. Disablism refers to prejudices against individuals who have suffered ‘mental, physical or sensory impairments’. In the past words such as lunatic, spastic and cripple were acceptable words used within health care settings as a way to describe an individual with specific needs. These terms are still rife within society today. (Walsh et al, 2005). Goodley (2011, Pg. 24) states â€Å"People with some form of impairment are likely to experience social disadvantage, a lack of opportunities and unfair discrimination. † Barton (2002) agrees with this as Barton theorises that individuals with a sensory, physical or mental impairment will at some stage in their life experience oppression in both institutional and individual forms, this can be due to absence of choices, barriers to funding, unforeseen deterioration in their impairment or general ignorance. Within the NHS discriminatory practices occurs on a regular basis despite programmes such as ‘Valuing People Now’ being released by the DOH in 2009, this was a three year strategy to help improve the quality of life for individuals with learning difficulties. This year MENCAP have released a report called ‘Death by Indifference: 74 deaths and Counting. ’ This report highlights the failings within the NHS to provide an equal quality of care for those with specific needs and learning difficulties. The main areas of failing that subsequently lead to the needless deaths of many patients with learning difficulties are failure to recognise pain, poor communication, diagnostic overshadowing, and delayed treatments, inappropriate DNR’s and lack of basic care. Throughout the report it is emphasised the lack of compliance not only to the Equality Act but also the Mental Capacity Act, in relation to DNR’s being allocated to patients without the consent or the knowledge of the patient’s family, friends or advocate. Again a major issue within the report is the NHS complaints and redress system in place. MENCAP,2012) â€Å"In this report we deal with the inadequacies of the NHS complaints process. On average it can take 18 months- 2 years to reach the Local stage, and between 2 years- 4 years to complete the ombudsman stage. † (MENCAP,2012, Pg. 7). Sexism is the belief that one gender is superior compared to the opposite sex, it is generally felt that women tend to be sexually discriminated against within society more so than men. (Walsh et al 2005). Within the health care profession a patient may express a preference in the gender of the health care practitioner (HCP) who provides their personal care (Smith t al, 2011). This is not to be seen as sexual discrimination against the HCP, by noncompliance with the patient’s request this can be viewed as discrimination against the patient’s religious and cultural beliefs. However due to under funding and under staffing with the NHS the ability for HCP to fulfil these requests has been compromised. â€Å"Modesty in dress and a requirement to be treated by a doctor/nurse of the same sex is also important in some religions. NHS staff should consider these requirements in order to preserve the dignity of the patient. However, it is not always possible or feasible to provide same-sex attendance, particularly without adequate notice that this might be an issue, and this should be made clear at the time of making appointments† (DOH, 2009) The Royal College recognise the need for a review within staffing levels within their 2011 report Mandatory Nurse Staffing levels, they also acknowledge the impact staffing levels have on a patient, they state, â€Å"There is a growing body of evidence which shows nurse staffing levels makes a difference to patient outcomes, patient experience, quality of care, and the efficiency of care delivered. (RCN, 2011). Within a female dominated profession such as nursing it is felt that males tend to be more so discriminated against sexually than females. A recent example of this on a national news level is the case of Andrew Moyhing, a male student nurse who won a sexual discrimination case against the NHS. He was told by a female staff nurse that he would need to be chaperone by a member of staff as he attached an electro-cardio machine to a female patient, due to ‘intimate care’ required . Mr Moyhing felt this was unfair as female students on the same ward were unsupervised when carrying out personal hygiene and intimate care on male patients. The charity ManKind this year released a report presenting the ‘Seven Challenges Male Victims Face’, it highlights various forms of sexism especially within statutory service such as the NHS, â€Å"Practically all training with in the police, NHS and local authorities is aimed at women as the victims, men as the perpetrators. † (See appendix 2). Cultural discrimination is when a s ociety does not accommodate or recognise the needs of an individual or group of people from a different religious or cultural background. As a society there will be expectations of what the ‘social norms’ are, what is acceptable and what is not acceptable. When an individual or social group show beliefs or needs that do not fit in with our own social norms they can be met with hostility and intolerance. (Walsh et al, 2005). â€Å"Cultural barriers can prevent, for example consideration of spiritual, relational or dietary needs that do not conform to traditional expectations. † (www. bridgingthegap. scot. nhs. uk). There are many areas of patient care which are can be jeopardised due to ignorance of cultural beliefs, examples of these which are emphasised in the NHS guidelines for cultural and religious beliefs are: Diet- many religions are restricted as to what types of food they can eat, and how the food was prepared, for example an orthodox Jew will not eat pork or any meal that has come into contact with a pork product, Muslims also have similar beliefs surrounding food preparation. Personal Hygiene- as mentioned before patients often express preference to the gender of the HCP who assist with their personal care. An orthodox Muslim will only wash in running water, therefore offering a basin of water to wash in may be seen as offensive. Palliative care and Dying- each religion has different views are to how a body should be ‘laid out’, when a patient dies the patient is given the Last Offices, if a patient is Christian a bible is also usually set in the room, however some religions find it offensive for anyone except the family to ‘lay the body out’. http://www. bfwh. nhs. uk). This is why more importance needs to be emphasised on patient admissions and filling in the ADL forms to the best of our ability, as it can prevent any future offence or discriminatory practices. As you can see in the Activities of Daily Living (ADL) (see appendix one) spiritual needs are mentioned, however the contents of the ADL will vary from trust to trust an d all trusts do not include religion or spiritual needs. In the caring profession patients have shown signs of frustration, confusion and anger as their cultural beliefs are not met, over looked or ignored. (Leininger, 1991). Discrimination based on sexual orientation, Roper, Logan and Tierney identify sexuality as one of the ADL (Mckenna et al, 2008), within each NHS trust it varies as to what is recognised as an ADL. As you can see (appendix one) the trust I work for have not included sexuality. Many people within society see sex as a taboo subject and therefore find it a difficult topic to approach and discuss openly, however sexual health is a part of holistic health and should be taken as seriously as any other aspect of health such as mental, emotional or physical. The World Health Organisation defines health as, â€Å"a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity. †(WHO, 1948). Studies have shown that the homosexual community are greatly disadvantaged and are the least likely group that identify with the protected characteristics to access health services. There are a variety of reasons due to this such as, a patient may not be openly gay, when they do access health services a majority of the time assumptions are made the reason for accessing that health service is related to the patients sexual health, also when homosexuals do access sexual health services they feel a stigma of promiscuity has been attached to the homosexual community. (Dunn et al, 2010). In conclusion from this essay I have realised barriers to health service access are a key factor in differential health outcomes among population groups within society. WHO, 2001) Although policies and procedures have been put in place by governing bodies to decrease the occurrence of discriminatory practice, these policies are not always effective; they are only effective when they are enforced by the organisation. Within the health care profession we must always strive to respect an all aspects of an individual’s identify and self-concept (Walsh et al, 2005). Simple measures such as filling in an ADL to the best of your ability can be a preventative method to causing future offence, or discrimination to a patient.

Technology and Society There is a town in eastern Pennsylvania called Centralia. It was a mining town fairly typical of Pennsylvania in the 1880’s. Today it is virtually a ghost town. Interestingly, it has not been degraded due to the decline in the demand for coal. The decline in population in Centralia is due to a mine fire that was started in 1962 and has still not been extinguished today. Many discussions these days of coal energy and society would include such topics as the pervasiveness of coal as an energy source in the world as well as the environmental impacts of using coal for energy.
